Online DBS Applications
You will need to log onto the following website:-

http://disclosure.capitarvs.co.uk/warwickshire/

Click on “Start Application” to enter the system and start your application.

Please note at this stage of the process your login details are case sensitive.

• Enter the Organisation Reference that has been supplied by us this is: Budbrookeprimaryschool
• Enter the Password that has been supplied by us this is: Warwick01
• Once you have completed this section click “Enter”

Once you have completed and submitted the application form you will then need to call back into the school office with your ID. You will need to produce one of the following:

• Passport
• Driving Licence
• Birth Certificate

And at least 2 from the following:

• Marriage Certificate
• Bank/Building Society Statement (Issued within the last 3 months)
• Utility Bills (Issued within the last 12 months)
• Council Tax Statement (Issued within the last 12 months)
• P45/P60 (Issued within the last 12 months)
• CRB Disclosure Certificate UK (Issued within the last 12 months)
